Version and course boundary
This course uses Laravel 13.x and assumes PHP knowledge: classes, interfaces, types, exceptions and Composer. The framework requires at least PHP 8.3; that does not prove an application and all its packages accept any later version. Before an actual change, the project contract and concrete dependencies also matter.
Here we analyze framework decisions through excerpts and scenarios, with assessment exclusively through answer selection.
Public entry and internal files
Server configuration should direct application requests to public/index.php. That file loads the Composer autoloader and obtains the application from bootstrap/app.php. Publishing the project root to work around a failing route exposes too broad an area: disabling directory listing does not block direct requests for known names.
Keep internal files outside the public root and diagnose server forwarding separately from application routing.
From boot to response
Boot prepares environment, logging, error handling and providers. The request passes through middleware and reaches its matched route if earlier controls permit it. Middleware can return a response without invoking the next component; in that case, lack of controller execution is expected.
When the controller returns a response, it travels outward through the middleware chain. Locate the last observed stage before attributing failure to business code.
One value and three representations
Distinguish the value written in .env, the external variable delivered to the process and the entry read through config. External variables can override the file. In the example, env is used in the configuration definition and config in the service.
The services.invoice.endpoint key identifies the services file and its inner keys; it does not include the config directory name. This separation allows discussion of effective values without scattering file reads throughout business code.
Caching and a destination change
config:cache gathers configuration values into a loadable artifact. Afterwards Laravel does not load .env for requests or Artisan commands; env can still return external variables. Therefore do not conclude that env always becomes unusable, and do not depend on it in services expecting values only from the file.
In the A-to-B scenario, the correction is rebuilding configuration with authorized inputs and observing the new execution. Restarting a process without changing an old cache can still load A.
Operation without publishing secrets
An .env.example with fictional values communicates expected inputs. A real plaintext .env can expose credentials even in a private repository; secret delivery needs its own controls. In production, confirm app.debug is effectively disabled, including the cache in use.
For application writes, align process identity and necessary access to storage and bootstrap/cache. Granting universal write access to the entire project is not a proportionate response to a logging error.
What a 200 demonstrates
Without extensions, the /up health endpoint indicates the application booted without an exception in that request. It does not establish that a database, billing integration or payment transaction is operational. A DiagnosingHealth listener allows adding checks, but their design should state what they observe and bound cost and time.
In the proposed case, the 200 and payment failure can coexist: the problem is treating a boot signal as if it covered the dependency required for admission.
<?php
// config/services.php: excerpt for reading, not a complete application.
return [
'invoice' => [
'endpoint' => env('INVOICE_ENDPOINT', 'https://billing.example.test'),
],
];
// In an application service, read the loaded configuration:
$endpoint = config('services.invoice.endpoint');The excerpt defines a configuration entry and separately shows the read performed by a service. The example.test domain is fictional. If configuration is cached with destination A, subsequently editing .env to B does not by itself change the value loaded by config.
Interpret the two locations; you are not asked to execute or publish this code.
Common pitfalls
Serving the project root; calling env inside actions dependent on dotenv; assuming an .env edit invalidates the cache; enabling production debug; treating /up as an automatic test of every dependency.

An application receives a request at a public entry point, loads configuration and applies middleware before the functional outcome. Diagnosis should compare intended and loaded configuration and compare check coverage with the operation to be admitted.
